Referral information for discharge planners and case managers
Everything you need to decide whether we fit your patient, without a sales call. If we can't take the referral, we'll tell you in one phone call rather than waste your discharge window.
What we accept

Why we're worth a call on a hard placement
- We cover outlying towns such as Halifax, Plympton, Carver and Rochester, which many agencies decline
- Three-hour visit minimum, lower than the usual four, so small-need discharges are viable
- Owner-led, so there's no queue between you and a decision
- We give a yes or no the same day, not "we'll get back to you"
- Cape Verdean Creole, Haitian Creole, Portuguese and Spanish speakers welcome on our caregiver roster
How a referral works
Call or email with the basics
Patient's town, discharge date, level of assistance needed, hours anticipated, and whether the family can fund private pay care. That's enough for us to answer.
We confirm same day
Whether we can staff it, when we can start, and the hourly rate. If we can't cover it, you hear that immediately so you can move on.
Assessment before or at discharge
We meet the family at the hospital or at the home, build the care plan, and confirm the schedule in writing.
Care begins
Typically within 24 to 48 hours of the referral. You get confirmation that the patient is covered.

What we can't take
Knowing this up front saves you a call on the wrong placement.
- Skilled nursing — wound care, injections, IV therapy, catheter management, medication administration
- Any case requiring a licensed clinician in the home
- Patients whose safety needs exceed what non-medical home care can hold, including wandering risk that requires a secured setting
- Cases we cannot staff at the hours required — we will say so rather than accept and under-deliver
We work alongside visiting nurse agencies regularly and are glad to be the non-medical half of a shared plan.
